What Happened?

According to the U.S. Department of Justice, between 2019 and 2023, Dr. Hares and others participated in a scheme that resulted in more than $7 million in fraudulent Medicare claims for psychotherapy services that were never provided. Hares and a co-conspirator signed purported patient progress notes that prosecutors said had actually been prepared by employees in Mexico rather than by the medical providers who allegedly performed the services.

The case began with federal charges announced in 2023. At that time, prosecutors alleged that Hares completed charts claiming he had seen Medicare beneficiaries for psychotherapy even though the appointments never occurred. Local reporting from ClickOnDetroit covered the original charges and described Hares as a West Bloomfield physician.

In April 2026, Hares was sentenced to 12 months in federal prison, ordered to pay $4.8 million in restitution, and ordered to serve three years of supervised release.

Michigan Medicaid records also show that Mustafa Hares MD PC, a medical group associated with Hares, received a summary suspension from MDHHS-OIG in September 2023.
