How to Request Medical Records for a Lawsuit
If you are pursuing a personal injury, medical malpractice, or product liability claim, obtaining your medical records is a critical step in evaluating and advancing your case. This guide explains how to access your records and submit them to our office quickly and securely.
How to Access Medical Records Through MyChart
Many healthcare providers use MyChart, an online portal that allows you to quickly access
and download your records.
Step 1: Log In
Access your provider’s MyChart via their website or mobile app. If needed, create an
account using your basic information or contact your provider for assistance.
Step 2: Locate the Document Center
Use the menu to find the “Document Center,” where records can be requested and
downloaded.
Step 3: Request Records
Select “Request Medical Records” and request complete records from the date of your
incident through the present.
Step 4: Download Records
Once available, download your records as PDF files. Be sure they include:
- Provider Notes
- Test Results
- Lab Reports
- Nurses Notes
- Flowsheets
- Input/output Reports
- Operative Reports
- Orders
- Medication Reports
- Anesthesia Reports
- Imaging Reports
Incomplete records may delay review of your claim.

All records should be sent to our Michigan office only, regardless of your location. If you must send original documents, use certified mail and keep copies for your records.
